Insurance
Patients are responsible for all medical charges; however, most primary care providers are honored at our office. If you have partial or complete insurance coverage, please call this to the attention of the personnel at the reception desk. On your first visit, our insurance personnel will discuss your particular insurance coverage with you and will then handle your claim for you. You will be required to sign a release of medical information in order to process any insurance claim for services provide to you by our office. You are ultimately responsible for the balance of your account for any professional services rendered within 30 days.
Private Pay
Private pay patients are required to pay in full at the time of the service.
Medical Records
Medical records are kept confidential by the office. Information is not released unless permitted by a signature from you.
